All students have been given their 2018-19 School Paperwork Package to take home. It is very important all forms are completed and returned to the school as soon as possible.
Included in the package:
- Student Verification Form – 2 sided, please indicate any corrections/changes and sign the back of the form
- Code of Conduct – complete and sign
- Emergency Release Form – 2 sided, please complete all areas (or make N/A) and sign the back of the form
- Community Resource Form
- Child Alert Form – please complete and sign
- Internet Policy – please complete and sign
- Personal Information Consent Form (Photo Permission Form) – please complete and sign
- Outside Media in Schools – please complete either the Consent OR Objection portion of this form
There are no Student Insurance Forms included with the package this year.
